What is Home Care?
|
Sutter VNA & Hospice provides high-quality health care that allows patients to recover, or live with illness, in the comfort of their own homes, surrounded by family and friends. By providing care, education, and support, we help patients become independent in managing their care.
Patients must be homebound, have a physician's order and insurance authorization to be eligible for home health services.
Examples of conditions which may require home care are: cancer, AIDS, diabetes, heart and lung diseases, fractures, and strokes, chronic illness, and disabilities.
A home care plan is tailored to each patient's specific needs by Sutter VNA & Hospice staff and the patient's physician.

The Home Care Team
|
Home care visits may be made by registered nurses, rehabilitation therapists, medical social workers, nutritionists, and home health aides, depending on the plan of care.
Skilled nursing care includes services and care that can only be performed safely and correctly by a licensed nurse.
Your doctor may order physical therapy, which includes exercises to regain movement and strength in a body area, and training on how to use special equipment or do daily activities.
You may receive speech-language therapy,which includes exercises to regain and strengthen speech skills.
Some patients receive occupational therapy, which helps you to become able to do usual daily activities by yourself.
We provide medical social services to help you with social and emotional concerns related to your illness.
Your home care team will provide physical, mental and psychosocial assessment; education assessment; medication teaching, treatments and procedures; safety assessment, and discharge planning.

Infusion & Pharmacy Services
|

We provide cost-effective infusion services to patients in their homes. Our specially trained infusion nurses and pharmacists coordinate professional services, pharmaceuticals, and supplies. Services provided include: Total Parenteral Nutrition, Antibiotics, Hydration, Chemotherapy, Pain Management, Enteral Nutrition and Injectable Medications. More about

Hospice Care
|

Sutter VNA & Hospice provides hospice care and support to terminally ill patients and their families, enabling patients to spend their remaining time in comfort and dignity in the familiar surroundings of their home, focusing on living. Specially trained hospice staff provide nursing care, pain and symptom management, spiritual support, bereavement counseling and support groups for patients and families. Who Pays for Care?
|
Home care services are usually paid for by Medicare, Medi-Cal and private insurance. There are certain medical and eligibility requirements that need to be met before Medicare, Medi-Cal or other insurance companies will authorize care. Our staff will work with you and your insurance company to coordinate benefits.
If you are uninsured, or if you believe your insurance and/or income will not cover the cost of your care, of if there is a change in your insurance, you may qualify for financial assistance. Sutter VNA & Hospice has a financial assistance policy for low-income uninsured patients who may not be able to pay for services. How to Arrange for Care
|
You, your family, or your doctor can call us when home care services are needed. We will coordinate your care with your physician, who supervises the services you receive.
