Rehabilitation Therapy

As part of our home health services, Sutter VNA & Hospice provides a wide variety of rehabilitation services to patients in their homes. These services include physical therapy, occupational therapy, speech therapy, and nutrition therapy.
The licensed professionals at Sutter VNA & Hospice will complete a comprehensive evaluation to determine the patient's functional status. Working with the patient's physician, an individualized treatment plan will be developed and carried out to achieve successful outcomes.

Referrals may be made by physicians, hospital staff, patients, community agencies, families and neighbors. We will contact the patient's doctor, who will order the specific care needed.
